Building Intelligence into India’s Healthcare Story

India’s healthcare story has often been defined by scale. The country operates one of the world's largest public health systems delivering care across urban centers, remote villages, and hard-to-reach districts through a network spanning hundreds of thousands of health facilities and supported by millions of frontline workers. As of March 2023, this network comprised 1,69,615 Sub-Centers, 31,882 Primary Health Centers, 6,359 Community Health Centers, and 714 District Hospitals. The next phase of progress will be shaped by how intelligently this scale sustains itself: by detecting health risks earlier, predicting epidemic outbreaks faster, shortening diagnosis timelines, and delivering care with greater precision.
Since 2022, driven by flagship government initiatives including the Ayushman Bharat Digital Mission, the National Health Mission, and the IndiaAI Mission, India has stepped up the integration of artificial intelligence (AI) into its healthcare delivery — recognizing that a system of this scale can only become more responsive, preventive, and equitable if it becomes more intelligent. Deployed across diagnostics, disease surveillance, telemedicine and national health platforms, this integration is helping the country’s health systems respond faster, detect disease earlier, and improve access to care. It is also creating the digital foundations that enable innovators, researchers, and healthcare institutions to develop solutions tailored to India’s needs. At its core, the integration is about building a healthcare system that is responsive, preventive, and equitable for every citizen regardless of where they live —a goal reflected in both the National Health Policy 2017 and the Viksit Bharat 2047 vision.

Access and Diagnosis at the Place of Need

At the center of this evolution is India’s steadily expanding digital public health infrastructure. For a country where a specialist physician can be hundreds of kilometers from the patient who needs one, access to good quality telemedicine is more than convenience; it is lifesaving.
Underpinning this advancement are government-led and government-run platforms including the Ayushman Bharat Digital Mission and e-Sanjeevani both of which are laying the groundwork for interoperable and connected healthcare systems nationwide. According to the Ministry of Health and Family Welfare, the government-run telemedicine platform, e-Sanjeevani has enabled over 476 million teleconsultations as of June 2026, helping improve the continuity of care and addressing specialist shortages across multiple states. Embedded within the platform is an AI-powered Clinical Decision Support System that assists doctors to arrive at quicker, clearer decisions through standardized data capture and AI-supported recommendations.
The same shift in improved efficiencies that telemedicine is enabling is also discernible in diagnostics and early healthcare interventions. Indian health-tech innovators are increasingly building for low-resource and high-volume environments, rather than limiting themselves to adapting tools designed for tertiary hospitals. This matters because India's disease burden is not located outside the doors of well-equipped urban hospitals. It is prevalent in Primary Health Centers (PHC), at sub-centers, and in communities, hours away from a specialist. Tools designed for these conditions have the capability to move the moment of detection and intervention significantly earlier in the lifecycle of illness, at a scale that can transform population-level outcomes. Some of the most promising examples emerge in screening and diagnostics, where technology is helping bring specialist capabilities closer to the point of care.
Take for example, Remidio's portable retinal imaging devices. These are enabling the facilitation of diabetic retinopathy screening closer to communities, reducing dependence on specialist-heavy, and often far more expensive, urban infrastructure. The company reports that its AI-powered solutions have screened nearly 7 million people globally for eye conditions including diabetic retinopathy, reaching some of India's most remote regions, from the hills of Himachal Pradesh to rural West Bengal. Similarly, Qure.ai’s AI-enabled imaging tools flag abnormalities in chest X-rays and other scans for TB, lung cancer, and neurocritical care in public health facilities.
InnAccel also exemplifies this design philosophy. Its portable neonatal CPAP device, SAANS, targets India's neonatal mortality rate of 19 per 1,000 live births. Deployed across 300+ public health facilities in Assam, outcomes data from 3,000 neonates show improved discharge rates and reduced mortality. Further, its Fetal Lite AI monitor was recognized at the 2023 edition of Goalkeepers event as capable of saving over 1,000 lives a day.
What distinguishes many of these innovative healthcare products is how they are designed to address the realities and constraints of healthcare delivery in India. In India, as in much of the Global South, healthcare innovation cannot take for granted uninterrupted electricity supply, advanced hospital infrastructure, or access to specialists at every point of care. Solutions instead need to be portable, affordable, durable, multilingual, and usable by frontline workers wherever they are irrespective of location and local conditions.
Scaling these tools to population-level impact requires institutional architecture: policy frameworks that build clinical trust, procurement systems that can deploy at scale, and last-mile delivery infrastructure. India is now building all three — through the IndiaAI Mission's Centers of Excellence, governance frameworks such as the ICMR Ethical Guidelines and the Strategy for AI in Healthcare, and 1.80 lakh Ayushman Arogya Mandirs now operational nationwide.

From Innovation to System Readiness

In March 2024, the Union Cabinet approved the IndiaAI Mission with a ₹10,371.92 crore budget, naming healthcare as a priority. Within a year, and directly under the Mission's framework, AIIMS Delhi, PGIMER Chandigarh, and AIIMS Rishikesh were designated Centers of Excellence for Artificial Intelligence.
Meanwhile, public health surveillance shows why the broader shift towards greater integration of tech into the healthcare space matters. The Media Disease Surveillance platform, integrated into the Integrated Disease Surveillance Program, has published more than 4,500 event alerts since April 2022. AI-driven surveillance and predictive analytics are demonstrating what earlier detection looks like in practice: identifying outbreak patterns before they escalate and enabling faster, more targeted public health responses.
The opportunity that this shift offers is significant but calls for caution and responsibility. Healthcare continues to play a defining role in shaping lives lived across diverse contexts shaped by geography, income, gender, digital access, and institutional capacity. The challenge, therefore, is not just about building smarter technologies, but ensuring that tech-enabled innovation enables good quality healthcare to reach everyone in the way required.
Beyond deployment, India is beginning to put governance instruments around its AI-enabled healthcare ambitions: establishing frameworks for ethical decision-making, clinical accountability, and trustworthy adoption. These frameworks are important because healthcare AI can earn public trust only if it is clinically valid, explainable, and accountable to people rather than just systems.
As innovation and AI move from pilots and point solutions into mainstream healthcare delivery, attention is shifting from technological capability to a responsible and equitable scale.
The country stands at a genuine inflection point the future of healthcare in India is poised to be smarter. Its real measure, however, will be whether it is also more equal, more trusted, and human in design; and whether it delivers on the promise the National Health Policy 2017 made, and the Viksit Bharat 2047 vision demands: quality healthcare for every Indian, wherever they are.
